NP Kim was not concerned about the pain she needlessly inflicted even after I let her know how much it was hurting. She continued on until I had a huge bruise on my forehead that can still be seen there. I kept on with the session because I needed the Botox for my migraines. I have gotten these treatments for several years without this kind of pain. She gave me my last Botox injections without this kind of treatment so I had no idea it would be this way with no concern for the way I was feeling.
I appreciated the time Dr. Sinclair took to ask questions regarding my concern and and explaining the steps he was recommending. It was a very positive experience and I would highly recommend him .
Shelcey Hussey R, NP was excellent. She gave me full freedom to express my thoughts and ask questions. In addition, she provided a very clear and understandable discussion of my condition and its management. Most unusual was her ability to make me sense that she cared for me.....that is an innate trait which is extremely valuable in health care interactions. As I left, I told her "you were made to be a primary health care provider". That says it all!
Shelcey Hussey was very kind and compassionate. Also very knowledgeable about what we should expect and what needs to be done going forward. She is awesome.

This was just my initial visit with the doctor. After doing some preliminary screening measures, he decided to have me come back to have an EMG done. Since we are not sure what is wrong with me, there was no treatment suggested at this visit. The doctor behaved in a professional manner during my visit, and he seemed interested in the things that I had to say to him about what has been going on with my feet and legs.
My provider, Christine Smith was simply excellent, all the way around. She spent a lot of time with me, discussing my MRI results, and how my diabetes and diet affects any futures TIAs (mini strokes), going forward. In the end, I really like seeing Christine, and feel I get the very best care from her. I can tell she truly cares about my health issues, and keeping me healthy, along with my family doctor. She is a great asset to your medical team.
Kim Fanguy listens to me and helps me to get well as opposed to just medicating me to mask symptoms. She takes her time and you can tell she really cares. That makes a huge difference in patient outcomes.

Usually very good. But NP rushed my injections. Causing them to hurt worse than usually and for multiple days. Also seeing random bump over my eyelids that I never had before in the 2 years of getting these injections before.

Dr Chowdary didn’t even address the reason I came in for. The visit was a total waste of time. He is NOT caring or concerned about you at all he shows no empathy at all!!!!! I left feeling so disappointed & don’t know what to do next.
She helped me understand that having a gag reflex is not a cause for concern, because it is not a change but the sensitivity I have always had. Also assured me that losing 5-10 pounds will not impact the dosage of my meds. Listened well and very responsive, she’s a KEEPER
Shelcey was very kind knowledgeable, well spoken and down to earth! She was a breath of fresh air. she actually taught us some things we didn't know about the stroke and what are our options are and how to be best equipped to minimize a recurrence or risk. The doctor was very straight forward which i appreciated but he spoke with me as if I already knew the answers and I felt as if he was just checking boxes.
Care provider made me feel physically uncomfortable and had virtually no bedside manner. Seemed to be rushing through my appointment and did not give clear instructions.
Very inconsiderate of the reasons I was there and had a list of questions. It was not able to have anything answered very unsatisfied. Very unsatisfied.

Prescription Refills
Call and leave a message with your name, date of birth, prescription and strength, along with your doctor’s name and your pharmacy’s name and number. Allow 24 to 48 hours for us to process the refill. We won't contact you about the request unless there is a problem.
We cannot refill some prescriptions.
You may be required to see your provider before we can refill your medication. We also do not accept refill requests from local or mail-order pharmacies—you must request these refills.
Test Results
You can receive timely notifications and view lab results from tests you’ve had done through MyChart. Test results should be available within 7 to 10 business days.
Allow up to 7 days for your referral to go through. Once it does, our office or the specialist's office will call you with your appointment information. If you make your own appointment, please let us know as soon as possible so we can obtain any prior authorizations that may be required.

Due to the volume of forms we process, please allow up to 10 business days for us to complete them. There may also be a charge for completing your form, or your provider may require that you come in for an office visit for assistance in completing your form.
